*Please make sure the signature is handwritten, not typed.

  • Full name of Parent/Guarding approving participation

  • Parent email address - different from participant email.

  • Participant School Name & Location

  • Current Grade Level, GPA, & Class Rank (if applicable).

  • T-shirt size
